Office Policies and Insurance Information

Our practice is dedicated to Cosmetic Dermatology & Laser Medicine. We do not accept health insurance for cosmetic procedures. Dr. Brozowski focuses exclusively on cosmetic improvement of the skin and aesthetic enhancement, and, therefore, does not treat medical conditions or skin pathology.  We suggest that all patients get a routine mole check and skin cancer screening once a year from their Primary Care Physician or traditional Dermatologist. 

To save time before your appointment, you may print and fill out the Cosmetic Registration History form and bring it with you. To view and print the Registration Form, you must have Adobe Acrobat installed. If you do not have it on your computer, you can download it for free off the Adobe web site. 

Cancellation Policy

We have a 48-hour cancellation policy for all appointments. To cancel or reschedule, please call us 48 business hours prior to your appointment time to avoid a fee (for example, for a Monday appointment, please cancel by the Thursday prior). This allows us to fill the appointment off of our waiting list. We do not cancel appointments via email -- please call the office! If you miss an appointment or do not call us at least 48 hours in advance, you will be charged a late cancellation fee of $95. We pride ourselves on having short wait times and a smoothly flowing office. Please help us maintain our schedule by being on time for your appointment. Patients who are 15 minutes late will be rescheduled.

Insurance Guidelines

Cosmetic Dermatology procedures are not covered by traditional insurance plans. However, procedures to treat acne or rosacea may be eligible for Flexible Spending Account benefits. Additionally, skin care products from our office are eligible for reimbursement from a Flex/Health Care Savings Account. Please let us know if you have a Flexible Spending Account, and we will prepare a reimbursement form for you to submit to them directly. Botox, Fillers and Laser Hair Removal are not usually eligible for FSA reimbursement.

Medical Records

If you would like your medical records transferred to another provider, we need this request in writing. Please download and sign the Medical Records Transfer Request and mail it to our office along with a $50 fee to cover the cost of photocopying and mailing, in accordance with California law. Our office will mail out a copy of the medical records of current patients within 15 days of receipt of this transfer fee. If it has been three years or more since your last visit, your medical chart has been moved to an off-site location, and it may take up to a month to locate and copy these chart notes. We do not maintain charts past seven years. It is our policy not to release medical records to anyone without an original patient signature.
