Filler injection, as an aesthetic procedure, has been used in the United States and Europe for two decades and continues to grow in popularity. Fillers are used to restore volume and fullness to the face and lips, and to correct wrinkles and deeper facial folds. Studies suggest that much of facial wrinkling and hollowing of the face is the result of a loss of facial volume, rather than sagging of the skin. Filler therapy will fill in wrinkles, restore volume to face and lift cheeks.
Dr. Brozowski also injects biostimulatory Sculptra. Sculptra is not a traditional fillers, but provides overall fullness for loss of volume associated with aging. Scupltra is Poly-L Lactic Acid and is a fruit acid that stimulates your skin to make its own collagen. Biostimulators promote new collagen creation for up to three months. Thus, the results last longer than traditional fillers. Sculptra is excellent for hollowing of cheeks and temples and the lower face. For vertical wrinkles in the décolleté, Dr. Brozowski uses Sculptra to soften and fill in lines to create a smooth surface.
NEW: We now offer Skinvive, a microdroplet hyaluronic acid injection for overall facial volume and softening of wrinkles, especially on the lower half of the face (cheekbone to jawline). Benefits
Fillers can smooth out folds and wrinkles by adding volume, which can make an enormous difference to your appearance. Filler injection can dramatically improve facial lines and wrinkles, depressed scars and a loss of volume in the face and lips. Unlike plastic surgery, the procedure enhances, but does not exaggerate, your natural features, and can be artistically sculpted to give results that vary from subtle to dramatic, depending on your aesthetic and treatment goal. Preventing Bruising
Here are some tips we share with out patients who have scheduled a treatment with injectables such as filler, Scuptra, Kybella or resorbable thread lift to decrease your chance of bruising. Bruising is a known and common side effect of Filler injections, so one must expect some degree of bruising with treatment.
To Prevent Bruising:
1. Discontinue all blood-thinners including baby aspirin, Ibuprofen, Aleve, Fish and (possibly) vitamin supplements. Avoid eating fish or taking fish oils for two days.
2. Eat a pineapple and/or take oral Bromelain the evening before and morning of procedure.
3. Do not drink wine or any alcoholic beverage within 24 hours of procedure.
4. Come in early to ice a full 15 minutes before procedure (note: this is not for pain management, it causes vasoconstriction of blood vessels. When vessels are smaller, there is less chance of injuring them with a needle and if punctured, they bleed less, resulting in less bruising).
5. Ice after procedure for a full 10-15 minutes in the office, and then as much as possible over the next 24 hours.
6. Start oral Arnica before procedure.
To Remedy Bruising
1. Ice. Do not push the ice into the filler, simply hold ice gently against skin. Ice frequently in the first 24 hours after treatment.
2. Take Ibuprofen - wait at least 2 hours after procedure to start, then take 2-3 200mg tablets every 6-8 hours for the next 3 days.
3. Take oral Arnica. We provide this with each treatment. Patients that run out of the Arnica but experience bruising may come by the office for an additional complimentary strip of Arnica.
4. Take Bromelain supplements, found at a Natural Pharmacy.
5. Use topical Auriderm post-op gel (available in our office).
6. Use Corrective Colors by Jane Iredale (available in our office).
Q&A
What are the results of filler therapy?
Smoothing out folds and wrinkles by adding volume can make a significant difference to your appearance. Dramatic results are immediately apparent and last several months to two years, depending on the filler used. All fillers will last longer with repeated treatment. In some facial areas, such as the brow and lips, Botox injection will make the filler last even longer. Touch-ups are recommended to maintain volume and prevent recurrent lines from forming. Properly applied, fillers enhance, but do not exaggerate features (as does plastic surgery) and do not effect normal facial expression.
What are the different filler options?
All the fillers used in our office are highly purified, have an excellent safety profile and are FDA approved. Hyaluronic acid fillers, such as Restylane, Restylane Silk, Juvederm Ultra, and Juvederm UltraPlus are medium-lasting fillers. These fillers are ideal for shallow wrinkles, the area between the brows, nasolabial folds (the facial lines around the mouth formed by smiling), volume in lips, the chin fold, as well as horizontal lines of the neck.
Restylane Silk, Kysse & Volbella are newer fillers introduced into the U.S. market, and are excellent for fine lines of the lips and for subtle lip volume.
Restylane Lyft, Restylane Refyme & Restylane Defyne, Vollure, Volbella and Voluma are longer-lasting fillers. They can last up to 12 months and are appropriate for deeper lines or an enhancement of the cheekbones,and the mid-face.
The Biostimulatory products, Radiesse & Sculptra, have a longer-lasting ability, with Radiesse 12-18 months and Scultpra up to two years. These are not appropriate for filling specific wrinkles or folds. Instead they are used to provide an overall fullness to hollow cheeks or a narrow face. Radiesse is also excellent for volume in aging hands, and Sculptra can be used to provide volume in buttocks or soften vertical wrinkles of the décolleté. Skinvive is the newest biostimulatory product, but unlike its predecessors, is injected superficially on the lower half of the face for cheek hollowing or crepe-y skin.
All fillers and biostimulatory products last longer with repeated treatments. It is recommended that you consult with Dr. Brozowski, who will determine what results you may achieve with each of the filler options and recommend the best filler enhancement for your facial contours and aesthetic.
What are the safety concerns and possible side effects?
Safety is of utmost importance. Therefore, cosmetic services are performed exclusively by the physician. Dr. Brozowski has performed more than a thousand filler procedures, without any permanent complications. Therefore, you can be confident that you are receiving the highest quality medical care. With all injectables, such as fillers and Botox, is especially important to have a licensed physician trained in Cosmetic Dermatology administer the procedure.
When used by an experienced doctor, the most commonly reported side effects are temporary redness and swelling at the injection site and/or bruising with some downtime. These typically resolve within a few days but the bruising may last longer. The use of ice, Ibuprofen, Arnica and bromelain can minimize swelling or bruising after the procedure, and are recommended.
What is collagen?
Collagen is a natural substance that provides strength and elasticity to the skin. Young, healthy skin contains an abundance of collagen and hyaluronic acid. As we age, we lose approximately 1% of our dermal collagen per year. This is exacerbated by overexposure to the sun and the loss of volume and fat beneath the skin. Where collagen diminishes, unwelcome lines, wrinkles and sagging skin begin to appear. Injectable fillers stimulate dermal collagen production in specific areas for a fuller effect. Biostimulatory products can cause a natural production of collagen for up to 3 months
Resurfacing procedures such as photofacial laser therapy, Clear+Brilliant laser treatments and medical microdermabrasion can increase the collagen level in your skin overall.
What is the level of discomfort during and after the treatment?
Depending on the area treated, topical or injected anesthesia may be used to minimize discomfort. Immediately after the treatment, you can expect a slight redness, swelling, tenderness or itching in the treated area. This is a normal result of the injection. The discomfort is only temporary and generally disappears in a few days. Patients are able to continue their daily routine after the treatment.
